• Valid Passport Required.
  • Passport must be signed
  • Over 6 months remaining validity
  • One blank Visa page available
• 1 form: Visa Form
• 2 Photos: Photograph Requirements
• Service Order Form

If Traveling on Tourism:

• Proof of Travel:

  Proof of Travel – (photocopy of roundtrip ticket, flight itinerary, or letter from tour agency/operator)

• Proof of Funds : copy of latest bank/creditcard statement
• Proof of Accomodations – (confirmed hotel reservations)
• Proof of Employment for verification and leave of absence

If Traveling on Business:

• Letter of invitation from host or sponsoring organization
• Business Letter Required: Business Letter Format OR
• If visiting family or friends:
  Invitation letter (original or copy via fax or email attachment) from the sponsor
  Sponsor’s legal status in Canada

• Proof of legal status for Non US Citizens:

  Proof of legal status

  • Valid US Visa (ie: H1B, F1, J1, E)
  • Advance Parole Document
  • Valid DS2019 (formerly IAP66)
  • Valid I-94
  • Valid I-20 (signed within 3 months by designated school official)
• Special instructions for minors:
  Photocopy of Birth Certificate
  Proof that the child is enrolled in school (if applicable)
  Letter of Consent from non-accompanying parent(s), notarized.
