• 2 Photos: Photograph Requirements
• Service Order Form

If Traveling on Tourism:

• Proof of Travel:

  Proof of Travel – (photocopy of roundtrip ticket, flight itinerary, or letter from tour agency/operator)

• Police Certificate
• Proof of Funds : copy of latest bank/creditcard statement or a letter from a bank.
• Letter from doctor stating traveler in good health and free of communicable disease and HIV test

If Traveling on Business:

• Business Letter Required: Business Letter Format

• Proof of legal status for Non US Citizens:

  Proof of legal status

  • Photocopy of Green Card
  • Valid US Visa (ie: H1B, F1, J1, E)
  • Advance Parole Document
  • Valid DS2019 (formerly IAP66)
  • Valid I-94
  • Valid I-20 (signed within 3 months by designated school official)
