  • One blank Visa page available
• 1 form: Visa Form
• 2 Photos: Photograph Requirements
• Service Order Form

If Traveling on Tourism:

• Proof of Travel

  Proof of Travel – (photocopy of roundtrip ticket, flight itinerary, or letter from tour agency/operator)

• Medical Record or
  Letter from doctor certifying traveler is in good health and free of communicable diseases.

If Traveling on Business:

• Approval from the Immigration Department and
  – Letter of invitation from host or sponsoring organization
  – Medical Record or Letter from doctor certifying traveler is in good health and free of communicable diseases.
• Business Letter Required: Business Letter Format

• Proof of legal status for Non US Citizens:

  Proof of legal status

  • Photocopy of Green Card
  • Valid US Visa (ie: H1B, F1, J1, E)
  • Valid I-94
  • Valid I-20 (signed within the last 3 months by a designated school official)
  • Advance Parole Document
  • Valid DS2019 (formerly IAP66)

Additional for Non-US Citizens:
Letter from employer stating date of hire and approved leave of absence.
