(Under Age 16)

PRINT THIS PAGE OUT and use this as your checklist for assembling the needed documents.

The DSP-11 form must be filled online at this link (https://pptform.state.gov/PassportWizardMain.aspx).
Follow the steps to apply for applying for a passport book and fill in the online form for a new first time passport. DON’T SIGN IT until instructed to by the postmaster.

 

Print out 2 copies of this DS-11form.
One will go inside the sealed envelope, one will be attached to the outside of the envelope for our reference.
The Post Office will charge you $35.00 to witness and seal these documents.
This link will help you locate local passport acceptance facilities- http://iafdb.travel.state.gov/ Call ahead for an appointment and tell them you need documents witnessed and sealed for a “hand carried passport”.

 

INSIDE THE SEALED ENVELOPE MUST GO: (all original documents will be returned)

2 PASSPORT PHOTOS

1 DS-11 APPLICATION FORM, SIGNED IN PRESENCE OF POSTMASTER.

1 ORIGINAL, STATE ISSUED BIRTH CERTIFICATE (with raised seal)

PARENTAL PERMISSION DOCUMENTATION, NOTARIZED DS3053 FORM (if applicable)

1 CHECK MADE OUT TO “DEPARTMENT OF STATE” FOR THE AMOUNT OF $160.00

1 PROOF OF TRAVEL/COPY OF ITINERARY (required for same day processing)

1 LETTER OF AUTHORIZATION (attached) completed and signed

 

THE PACKAGE YOU WILL SEND TO US:

1 SEALED ENVELOPE FROM POSTMASTER (containing all the above documents)

1 EXTRA COPY OF DSP-11 APPLICATION FORM

1 TRAVEL DOCUMENT EXPRESS SERVICE ORDER FORM

1 LETTER OF AUTHORIZATION

Total fees are:
$600 Same Day (or $350 for 2 week process) our service fee (paid by credit card provided on the TDE charge form)
$35 postmaster fee (paid to the post office when documents are sealed)
$160 passport agency fee (paid by check inside sealed envelope)
$28-$30 weekday return overnight shipping fee. (waived if picked up locally)
