e. Court Order of a Name Change
PROOF OF TRAVEL:
A copy of the airline ticket or travel itinerary demonstrating your impending departure within ten (10) business days (if no visa is required) or within a month if a visa is required of the date of application.

2 Week process $368.32
SHIPPING FEE: $25 -$30 via Fedex (Weekday Delivery) or $35-$50 (Saturday Delivery).
