- Each minor child shall appear in person.
- For security reasons, parental consent may be requested.
- At least one parent must accompany the applicant (age 16-17).
Please gather the following documents
COMPLETE AND PRINT TWO (2) COPIES OF THE ONLINE DS-11 FORM
Please do NOT sign the DS-11 application form until the Passport Acceptance Agent instructs you to do so.
Because State Department requires that the application (DS11) has to be submitted within 5 days of when it is dated, please be sure that this form is dated by an acceptance agent no earlier than the day before you overnight your documents.
PROOF OF US CITIZENSHIP:
- Certified U.S. Birth Certificate; or
- Previous fully valid US Passport; or
- Report of Birth Abroad (Form FS-240); or
- Certificate of Birth Abroad (From DS-1350); or
- Certificate of Citizenship or Naturalization from USCIS.
PROOF OF IDENTITY(e.g. driver’s license, greater than 6 months)
*If your child does not have identification of his/her own, you need to accompany your child and present identification.TWO IDENTICAL PHOTOGRAPHS
*Effective November 01, 2016, customers must remove glasses for their passport photo.PAYMENTS(Total of $205 Effective April 02, 2018) in the amount of:
- $170 ($110 + $60 rush fee) for individuals 16 years and above to satisfy the U.S. Passport Agency AND
- $35 charged by your local clerk of the court or postmaster to execute this application.
PROOF OF TRAVEL:
A copy of the airline ticket or travel itinerary demonstrating your impending departure within ten (10) business days (if no visa is required) or within a month if a visa is required of the date of application.
Take all these to your postmaster or local clerk of court
ADVISE THE ACCEPTANCE AGENT YOU ARE APPLYING FOR A RUSH “HAND CARRY” PASSPORT APPLICATION.
THE ACCEPTANCE AGENT WILL WITNESS YOUR SIGNATURE ON THE DS-11 FORM.
Please be sure that this form is dated by an acceptance agent no earlier than the day before you send your documents.
YOU MUST THEN REQUEST OF THE ACCEPTANCE AGENT THAT THE ABOVE DOCUMENTS BE PLACED IN AN ENVELOPE, SEALED AND NOTED:
“To be opened by Passport Authorities Only”.
ENCLOSE IN A SEPARATE FEDERAL EXPRESS OR UPS ENVELOPE:
THE ABOVE NOTED SEALED ENVELOPE CONTAINING YOUR APPLICATION.
PHOTOCOPIES OF THE COMPLETED DS-11 FORM, PROOF OF TRAVEL AND LETTER OF AUTHORIZATION.
