NY Visa Form for residents of all other states.

• Proof of Travel:

  Proof of Travel – (photocopy of roundtrip ticket, flight itinerary, or letter from tour agency/operator)

Proof of Accomodations – (confirmed hotel reservations)

If Traveling on Business:

• Letter of invitation from host or sponsoring organization
• Business Letter Required: Business Letter

• Proof of legal status for Non US Citizens:

  Proof of legal status

  • Photocopy of Green Card
  • Valid US Visa (ie: H1B, F1, J1, E)
  • Valid DS2019 (formerly IAP66)
  • Valid I-94

Information must be typed in on the visa form as applications completed by hand are not accepted.
