Non US Visa Requirements:

• Original Passport
• 1 form: Visa Form
• 2 Photos: Print name of applicant on the back of it. Photograph Requirements
• Service Order Form

If Traveling on Tourism:

• Proof of Travel

  – Actual travel tickets (confirmed or open-dated) or
  – A written confirmation from an airline or travel agency that onward travel has been booked and paid for)

• Proof of Fund: NZ$100 per person per month of of the visit
  One original plus copy of latest bank/creditcard statement
• Proof of Accomodations – (confirmed hotel reservations)
• Sponsor’s Declaration Form, if applicable.

If Traveling on Business:

Business Letter Required: Business Letter Format

• Proof of legal status for Non US Citizens:

  Proof of legal status

  • Original Green Card or
  • Photocopy of Green Card, both sides, notarized
  • Valid US Visa (ie: H1B, F1, J1, E)
  • Advance Parole Document
  • Valid DS2019 (formerly IAP66)
  • Valid I-94
  • Valid I-20 (signed within 3 months by designated school official)
