An Authorization issued by the Directorate General of Immigration in Indonesia will be required to enter the regions of Maluku, Poso and Irian Jaya/West Papua.

Visa Requirements:

• Valid Passport Required.
  • Passport must be signed
  • Over 6 months remaining validity
  • One blank Visa page available
•  2 forms: E-Visa Form(New!)
• 2 Photos: Photograph Requirements
• Service Order Form

If Traveling on Tourism:

• Proof of Travel

  Proof of Travel – (photocopy of roundtrip ticket, flight itinerary, or letter from tour agency/operator)

• Proof of Funds : copy of latest bank/creditcard statement
• Proof of Employment for verification and leave of absence

If Traveling on Business:

• Letter of invitation from host or sponsoring organization describing the
  purpose of the visit and
  Photocopy of Inviter’s photo ID
• Business Letter Required: Business Letter Format AND
  Proof of Travel: Flight itinerary
  Proof of Funds: Bank Statement

• Proof of legal status for Non US Citizens:

  Proof of legal status

  • Photocopy of Green Card
  • Valid US Visa (ie: H1B, F1, J1, E)
  • Advance Parole Document
  • Valid DS2019 (formerly IAP66)

Special instructions for minors:
-Notarized copy of Birth Certificate
-Notarized Letter of consent from non-accompanying parent(s).
